48-Hour Fast Schedule: Two-Day Plan, Milestones, Refeeding Guide
A 48-hour fast doubles the autophagy window of a 24-hour fast and starts dropping inflammation markers. It's a serious commitment — two days without food — but the metabolic and cellular benefits are well documented. Done once a month, it's a high-leverage protocol.
How 48-Hour Fast works
Last meal Day 1 around 19:00. Skip all eating on Day 2 and Day 3. Break fast on Day 3 around 19:00. Water, sea salt, magnesium, potassium, black coffee, plain tea. Plan Day 2 as a light activity day, Day 3 as a mostly-rest day.
A sample day
- Day 1, 19:00Last meal: balanced, low-sugar, satisfying.
- Day 2, AM/PMWater 3 L. Sodium 2 g. Magnesium 300 mg. Potassium 600 mg. Walks.
- Day 2, 22:00Salt water before bed for sleep quality.
- Day 3, AM/PMSame electrolyte plan. Lower energy is normal. Be gentle with yourself.
- Day 3, 19:00Break fast with broth, then a small refeed meal (see below).
- Day 4Continue careful refeeding through the morning before normal eating.
What happens in your body
Physiological milestones reached during a 48-hour fast.
- h4
Blood sugar normalizes
Insulin drops back to baseline as your last meal finishes digesting.
- h8
Glycogen burning begins
Your liver starts releasing stored glycogen to keep blood glucose steady.
- h12
Glycogen depletion
Liver glycogen runs low; the body shifts toward fat for fuel.
- h14
Fat burning accelerates
Lipolysis ramps up — stored triglycerides break down into free fatty acids.
- h16
Mild ketosis starts
The liver begins producing ketones from fatty acids. Mental clarity often improves here.
- h18
Growth hormone rises
HGH increases up to 5×, supporting muscle preservation and tissue repair.
- h24
Autophagy begins
Cells start digesting damaged proteins and organelles — the housekeeping phase.
- h36
Deep ketosis
Beta-hydroxybutyrate climbs; the brain runs primarily on ketones.
- h48
Inflammation drops
Inflammatory cytokines fall; insulin sensitivity improves.
What to eat
Refeed in two phases. First meal (19:00 Day 3): bone broth or vegetable broth, wait 30 minutes, then a small portion of cooked vegetables and lean protein — half what you'd normally eat. Second meal (Day 4 breakfast): a normal-sized balanced meal. Resume regular eating by lunch Day 4.
How to break the fast
- Bone broth, then a small portion of scrambled eggs with steamed spinach
- Miso soup, then steamed white fish with cooked carrots
- Vegetable broth, then half a cup of dal with white rice
- Plain Greek yogurt with a stewed apple
Who it's for
- • Experienced 36-hour fasters
- • People targeting deep autophagy and inflammation reduction
- • Those building toward 72-hour fasts
Benefits
- • Deep ketosis (β-hydroxybutyrate >2 mmol/L)
- • Autophagy fully active
- • Inflammatory markers drop by hour 48
- • Insulin sensitivity rebounds noticeably for days afterward
Risks & cautions
- • Significant electrolyte management — daily sodium, potassium, magnesium
- • Cold sensitivity and lightheadedness common
- • Sleep often disrupted on the second night
- • Refeeding must be careful — go slow
- • Not safe for diabetes, eating-disorder history, pregnancy, or many medications
